Physical Therapists and Wheelchair Fit: Why the Right Seating Matters

Every September 8th, the physical therapy community marks World Physical Therapy Day, a moment to recognize the clinicians who help people move, function, and live more independently. For wheelchair users, few professionals matter more. A PT's trained eye during a seating evaluation can be the difference between a wheelchair that merely transports someone and one that actually supports their health, posture, and daily participation in life.

Seating and mobility work rarely makes headlines, but it's some of the most quietly impactful clinical work out there. This year, we wanted to take a moment to talk about why that work matters so much, and to recognize the therapists who've helped shape it.


More Than a Seat: How Positioning Affects the Whole Body

It's easy to think of a wheelchair seat as just a place to sit. In reality, seating and tilt configuration ripple outward into nearly every system in the body.

Posture and alignment. Poor seat positioning can lead to pelvic obliquity, spinal curvature, and asymmetrical loading over time, issues that compound the longer they go uncorrected. A well-fitted seat helps maintain a neutral, supported posture that reduces strain on the spine and joints.

Breathing. Trunk position has a direct effect on respiratory function. When the pelvis and spine are properly aligned, the diaphragm and rib cage have room to move as they should. A slumped or poorly supported posture, by contrast, can restrict lung expansion and make breathing measurably harder, a particularly important consideration for users with respiratory or neuromuscular conditions.

Skin integrity. Pressure injuries are one of the most serious risks for full-time wheelchair users. The right combination of cushioning, tilt, and weight distribution redistributes pressure away from vulnerable areas like the sacrum and ischial tuberosities. PTs help users learn to shift their weight and maintain good blood flow in the at-risk tissue. Tilt-in-space, in particular, gives users who do not easily reposition themselves, a way to periodically offload pressure without needing to transfer out of the chair. That is great for both skin health and independence.

These aren't separate concerns. Posture, breathing, and skin health are interconnected, which is exactly why a proper seating evaluation looks at the whole picture rather than any one measurement in isolation.


What PTs Look For in a Fitting

A skilled seating evaluation goes far beyond taking a few measurements. Physical therapists typically assess:

  • Pelvic position and stability, is the pelvis level, and can it be maintained in a neutral position under load?

  • Range of motion and flexibility, what postural corrections are realistic given the user's individual anatomy and any fixed contractures?

  • Trunk and head control, how much external support is needed to maintain functional alignment?

  • Pressure distribution, where are the areas of highest risk, and what cushioning and tilt strategy addresses them?

  • Functional goals, how does the user need to move, reach, transfer, and interact with their environment day to day?

This is where adjustability becomes essential. A chair that can be fine-tuned , seat-to-back angle, tilt range, depth, footrest position, gives the PT the flexibility to translate their clinical assessment into a precise, individualized configuration, rather than settling for the closest available preset. Participant's adjustability was built with exactly this kind of clinical decision-making in mind, giving therapists the range they need to fit the chair to the person, not the other way around.
